Scribenote is a software used to record and transcribe conversations during veterinary visits. The Scribenote software is utilized by veterinary service providers and clinics (“we” or “us” or “our”) to enhance the quality of services provided and to assist in effective and accurate documentation of pet information, symptoms, assessments or examinations and treatment plans. If pet owners provide personal information during veterinary visits, this information would also be recorded and transcribed through use of the Scribenote software. We do not use or disclose any personal information or other information outside of what is necessary to provide the services to your pet, and always in accordance with our privacy policies and applicable privacy laws. If you have any questions about our information and privacy practices, please contact Animal Clinic and Hospital at 432-694-2537. your decision to consent to use of the scribenote software during veterinary visits for your pet(s) at the below named veterinary service provider or clinic is completely voluntary.
